Fusion Gems

At level 50 you get a quest to go talk to Whitney. If you don't get a quest, just go talk to Whitney.
You can find her on the bridge just past the perfumer. Or you can just open your minimap and search "Whitney". The link will autoroute you to her.

She sells the fusion gem recipes and the materials. The recipes require certain levels of alchemy to learn. Recipes level 1 & 2 require alchemy 5; recipes level 3 & 4 require alchemy 6; and recipes level5 & 6 require alchemy level 7.

The catalog can be gotten from trading in Ode to Guardian points at the Gem and Rune merchants, from Whitney once you reach 120 dedication, and from the fusion gem menu in your character screen if you reach 280 dedication.

Your learned recipes are in your alchemy menu. Look at the recipe ingredients to see what you need.

All the materials can be bought at Whitney if you have the right items. The gem tokens that Whitney asks for in one of her tabs came from orbs. The catalysts only come from her shop as far as I know. The Mana Origin Stones do drop in Free Isle Invasion instance - specifically the naga one.

The easiest way to obtain the mana stones is through catalogs bought with ode to guardian (especially since they mucked up the ded). You get ode to guardian from roll-boxes and there are plenty of roll-boxes to be found: daily login roll, Fate, Polarland Invader, Free Isle Invasion, gear instances.

The other materials you can find in gear instances:

Peace Lily: Deepsea Gateway (in the circle of the second boss, top of the hill right before you turn for the third boss. Botany 1)
White Stone and Whitedrop Stone: Lost Lighthouse (appears after you defeat the final boss. Mining 1)
Alven'shar: Welkins (in the area at the first boss, just past the second boss, and at the third boss. Botany 3)
Violet Stone and Violetdrop Stone: Welkin (appears after you defeat the trio boss. Mining 2)
Venomfog Mushroom: OAC (along the path to the third boss. Botany 6)
Blue Stone and Bluedrop Stone: OAC (appears after you defeat the final boss. Mining 4)
Decayed Moss: AC (in the first room, and along the path to the 4th boss. Botany 6)
Gold Stone and Golddrop Stone: AC (appears after you defeat the final boss. Mining 4)
Glacier Blood: FP (in the passage after the first boss. Botany 7)
Crimson Stone and Crimsondrop Stone: FP (appears after you defeat the final boss. Mining 5)
White Mushrooms: ToK (in the room after the first boss. Botany 7)
Dream Stone and Dreamdrop Stone: ToK (appears after you defeat the bonus boss - you must complete the 5 challenges to open the door to the bonus boss. See THIS for details. Mining 5)

Collect the items, make the gems, find them in your bag, right click them to place them into your fusion menu. You get a stat increase - permanent resistance - with each placement and a bonus increase upon completion of a row.

You do not need to complete a whole row in your fusion gem menu to add to another row. You can skip rows if you want to. If I could not get materials to finish a row, I moved on to the ones I had materials to complete.

These cost resources (collected materials, catalogs, vigor), and some things are harder to come by than others, so I usually pick and choose which I want to do first, but ultimately you will want to fill the whole thing.
